How to apply for admission.

1. Please email wleenman@fortysevenmain.com or call toll free 800-287-5325 to see if we have a vacancy. This is also a good time to contact us to see if this would be an appropriate referral. We do not work with all diagnoses and we may recommend another program that might better address your needs.
2.Download an application, fill it out and send it back to us.
3.Ask that pertinent records be send to us. This would include hospital admission(s) and discharge summaries, psychological tests and vocational tests you may have had, psychiatrist and/or psychologist’s notes, etc.
4.We will review your records and if it appears that you would be a good candidate, we will contact you to schedule an interview and/or an overnight visit. That way you will have a good way to see our program and see if our program will address your needs, and we will have a chance to meet you.

Notice: All incoming residents must sign release of information so that we can coordinate vocational, testing, medical and psychiatric services.
